Chronic respiratory failure is the result of a group of diseases that cause airflow obstruction and breathing-related problems. This condition most often includes emphysema and chronic bronchitis.
Emphysema is a form of chronic obstructive pulmonary disease (COPD) defined by abnormal and permanent enlargement of air spaces away from terminal bronchioles and associated with destruction of alveolar walls. Emphysema causes shortness of breath through airflow limitation, hyperinflation, and loss of gas exchange surfaces in the lungs (also known as increased physiological dead space).
Endoscopic lung volume reduction (ELVR) with Endobronchial valves are a form of minimally invasive procedure for treating people with severe emphysema. Yis administered as a treatment option for carefully selected patients. ELVR with endobronchial one-way valves (EBV) has been shown to improve lung function, exercise capacity, and quality of life in patients with heterogeneous and homogeneous emphysema without collateral ventilation.
→ The endobronchial valve is an implantable device designed to occlude bronchi in diseased areas of the lung and allow air to be exhaled from the treated lung lobe.
→ELVR with endobronchial valves is performed with minimally invasive methods thoracoscopic (VAT) and robotics (RATS). Using positive pressure mechanical ventilation and a flexible endotracheal tube or a rigid bronchoscope, endobronchial valves are placed and deployed in the airways. The whole procedure takes about an hour and usually the stay in the clinic is 48 hours.
→ It is a safe and painless procedure that has numerous benefits for the patient. Pneumothorax is a common complication and usually occurs in the first 2 days after treatment. Long-term sequelae from EBV treatment do occur, but are easily managed. However, ELVR is not suitable for everyone and a thorough assessment is required before this treatment is offered.
